Please fill out the COVID-19 form as well as the form corresponding to the service you will be receiving.

Please note: The service forms only needs to be filled out once, the COVID-19 form however, needs to be filled out prior to every appointment until further notice.

COVID-19

Things we're doing to contribute:

​

• We wear a mask from the minute we greet you at the door, to the moment you leave​

• The studio is cleaned thoroughly at the beginning and end of each day

• The table has a non-porous vinyl covering

• The table, along with all touch points are thoroughly sanitized between each appointment with a medical grade disinfectant (PreEMPT CS20 or Cavicide)

• As usual, all tools are cleaned, disinfected and sanitized for each client

• We are washing and sanitizing our hands thoroughly throughout the day

• We've moved all of our consent forms and waivers to an online platform to reduce touch points in the studio

• We carry extra masks and have hand sanitizer readily available

Things we ask you to do to contribute:

•  If you have travelled outside of Australia or have been in contact with someone who has, please reschedule your appointment

• If you are feeling ill, or have been in contact with someone who is sick (ANY type of sickness), please reschedule your appointment

• If you have been in contact with someone who has tested positive for COVID-19 or is a probable case, please reschedule your appointment

• We ask that you wear a mask into the building and that it remains on for the duration of your appointment

• The morning of your appointment we ask that you do a self assessment and fill out the COVID-19 form provided in your reminder email. Honesty is the best policy - please answer these questions truthfully

• Please do not bring guests to your appointment. This includes children

• Please limit the number of items brought into the studio with you. Anything you bring in will need to be kept on the table with you

• Upon arrival, you will be asked to sanitize your hands

• If arriving early, please text us before coming up as we can only allow one client in the studio at a time
